Health Information & Medical Records


This department is responsible for maintaining adequate and accurate patient information and medical records on every patient that is seen at El Campo Memorial Hospital, as an inpatient, outpatient or emergency patient. The Health Information Department (HIM) serves to provide a central file of medical records compiled during evaluation and/or treatment of a patient which will be used as a permanent record. They are also responsible for coding diagnoses and procedures for reimbursement.

The hours of the HIM Department are Monday through Friday from 8 a.m. - 5:00 p.m. The HIM Department handles all requests for medical records and Advance Directives (Living Wills). Copies of medical records must be requested in person, due to confidentiality laws and HIPPA guidelines. A 48-hours advance notice is recommended for medical records requests, unless there is an emergent situation.

Patient Billing: Various physicians involved in your care at ECMH will bill you directly for their services. These may include your anesthesiologist, emergency room physician(s), radiologist, pathologist, cardiologist or other consulting physician during your ER visit or hospital stay. Please remember that some physicians are private practitioners (contracted by ECMH and not an actual employee of ECMH) and may not be a provider on your specific managed care plan.
