Swing Bed Program
If you have been hospitalized and are being discharged to a rehabilitation facility, you have the choice to come to El Campo Memorial Hospital for that REHAB Care!
The purpose of the Swing Bed Program is to allow a patient to stay in the hospital for a longer period of time and be under the observation and care of a physician and nurse.
How It Works
The ECMH Discharge Planner and a Physician will evaluate the patient and if certain criteria are met, the patient/or significant other will be given the opportunity to decide to transfer over to this program. Current ECMH patients will not be changing rooms and the care provided will not be disrupted. Out of town hospitalized patients can be transferred to the ECMH Swing Bed Program by contacting the Discharge Planner.
ECMH's Swing Bed Program is to assist patients in improving their physical, emotional and social function to become as independent as possible. Daily activities are provided to patients in the Swing Bed Program. ECMH Swing Bed Program is an alternative to short term Nursing Home Rehabilitation Care.
ECMH Swing Bed is a Medicare sponsored program that "swings" patients from acute care to skilled care who need temporary and transitional care.
